Game score appeal!

I was going to annotate one of my best games for you, a win against International Master Colin Crouch from the Durham Open in 2000. Unfortunately, I've misplaced the game score.

I've scanned online, but am struggling to find it. If anyone knows where I can pick up the pgn, please let me know.
