Friday chess puzzle 13

Boris Spassky v Marszalek, Chess 1958

Our position today is from the World Students' Team Championship 1958, and future World Champion Boris Spassky about to win against Marszalek.

There are two questions:

1. Which move did Boris, White, play (it forces mate in 5)?

and

2. Does anyone know Mr Marszalek's first name?
